Dr. J Gomez and his colleagues used electron microscopy and immunohistochemistry to clarify the theory that alveolar soft parts sarcoma exhibit myogenic differentiation 3. Nineteen alveolar soft parts sarcomas had typical light microscopic features. The diagnostic ultrastructural feature consisted of intracytoplasmic crystals. Immunohistochemical staining with antibodies to MyoD1 and myogenin, regulatory, nuclear phosphoproteins of the transcription factor that determines smooth muscle differentiation, was performed. There was an absence of nuclear staining for MyoD1 and only minimal scattered nuclear staining was present for myogenin. Granular staining of the cytoplasm of the alveolar soft part sarcomas for myoD1 and myogenin was present. Since, only nuclear staining is specific for skeletal muscle differentiation, this immunohistochemical and ultrastructural study of nineteen alveolar soft parts sarcomas does not support the theory of myogenic differentiation in this type of sarcoma.
