Case 2: Adenovirus Infection in Liver

Case Presentation

This 3 year old male patient had relapsed ALL (acute lymphoblastic leukemis) and received a bone marrow transplant. Two months after the transplant, he developed a fever and hepatitis.

Lab findings were:
SGOT, 633 IU/L; SGPT, 223 IU/L, GGT, 419 IU/L, and total bilirubin, 0.3 mg/dl.

A liver biopsy was performed, which showed periportal necrosis and multiple eosinophilic intranuclear inclusions (Figure 3) compatible with adenovirus in the hepatocytes. Stool and throat swab samples were positive for adenovirus.

Electron microscopy showed degenerate hepatocytes with nuclear and cytoplasmic adenovirus inclusions.

The viral particles were generally dispersed, but occasional pseudocyrstalline arrays were seen . The particles measured approximately 70 nm in diameter.
